Drug Policy : If only non-prescription items are being ordered, no license information is necessary.
Regulations require us to limit the sale of prescription and controlled substances only to registered, licensed healthcare professionals. All new customers must furnish STAT Pharmaceuticals, Inc., via Fax, with current copies of their state registration (license) and DEA certificate, verifying the requested ship to address. Controlled substances can only be shipped to the name and address exactly as it appears on the DEA form submitted. If no DEA certificate is submitted, no purchase of controlled drugs (class II - class V) will be permitted.
The Drug Abuse Control Act requires that you use DEA Form 222 for all orders of Schedule II pharmaceuticals. When ordering Schedule II drugs, please note the following:
• Orders can be shipped only to the address on the form.
• No alterations, erasures, or errors can be accepted.
• Retain copy #3 for your records.
• A current copy of your DEA Registration Certificate must be in our possession.
• The Form 222 must be mailed; a fax can allow us to make sure we have the product on hand for you, but we cannot ship until we physically take possession of the form. Faxing the form also saves time in that we can spot errors promptly instead of finding out the errors several days later and then asking you to fill out and mail another one.
• Please order far enough in advance to allow for the mailing of the Form 222.

Credit Policy : All online customers must place their order by credit card. STAT Pharmaceuticals, Inc. currently accepts VISA and MasterCard.
Shortages & Discrepancies : All shortages or discrepancies of shipped merchandise must be reported to STAT Pharmaceuticals, Inc. within 72 hours of package receipt. Please see returns section for where to call.
Returns : Items purchased from STAT Pharmaceuticals, Inc. are eligible for return if a request is made within 15 days of invoice date. For a full credit, if the merchandise you received is defective or not as you ordered, please call our customer service team at 1-800-748-5665 and request written authorization for return. Requests for returns after 15 days are subject to a 25% restocking fee.
The merchandise to be returned must meet the following criteria:
• Product must be in new, re-saleable, and unmarked condition.
• Was not a special order.
• Unless defective, product must be unopened in original packaging.
All return merchandise will be inspected before any final decisions are made on credits. STAT Pharmaceuticals, Inc. will reimburse proportional shipping charges on items that were shipped in error or defective upon arrival. All returns that pass the above requirements will entitle the purchaser to a credit issued to the same credit card account that was charged in the original purchase.
To ensure prompt return processing, please include the original packing list, invoice or RA number in with the return. Refunds will be for merchandise value only. Please allow three to four weeks for return processing, although turnaround time is normally quicker.
Unauthorized Returns : Except for STAT error, the following shall be considered non-returnable merchandise and accordingly, no credit will be issued and all such items returned will be destroyed:
• Controlled Drugs
• Expired Products
• Items not returnable to the original manufacturer
• Items returned without proper authorization
• Refrigerated or frozen items
Shipping Policy : All orders will be shipped Federal Express ground except for refrigerated products, which will be sent next day air. Shipping charges are prepaid and added to the invoice. At checkout you will be allowed to choose the shipping method and will see how much the freight charge will be. If you order items that are noted as hazardous materials, a flat $16.00 Hazmat charge will be assessed, although it will not show at checkout. * Effective September 1, 2005 a $6.00 fuel surcharge will be added to your order at the time of invoicing.
Sales Tax : Customers in California will be charged 8.75% sales tax or more, depending on your county tax rate... unless a copy of their sales tax exemption certificate has been previously sent to STAT Pharmaceuticals, Inc. Any applicable tax on sales to other states is the responsibility of the purchaser.
Minimum Order : All orders under $75.00 will be assessed a $20.00 below minimum order fee in addition to the shipping charge.
In Stock Policy : In the event that a product you order is out of stock at the time of shipment, we will contact you with the estimated shipping time and allow you to decide if you′d like to wait or purchase an alternate product or obtain it elsewhere.
Additional Terms and Conditions : All sales transactions shall be deemed to be fully negotiated, transacted and performed within the County of San Diego, State of California, U.S.A. All sales transactions and sales agreements shall be governed by and construed according to the laws of the State of California. In the event of any dispute concerning a sales transaction or sales agreement, including, without limitation thereto, Customer′s obligation to pay for goods sold by STAT Pharmaceuticals, Inc., the proper forum for legal or equitable claims and/or proceedings shall be either the General District Court of San Diego County, California, U.S.A., and to this end, STAT Pharmaceuticals, Inc. and Customer agree to subject themselves to the jurisdiction of said court in the event of any such dispute. STAT Pharmaceuticals, Inc. hereby reserves the right to amend these Terms and Conditions at any time.
